CONFERENCE DESCRIPTION

Our conference on Current Issues in OB/GYN has been designed for physicians, certified nurse midwives, nurse practitioners, PAs, nurses, and other healthcare professionals who face ever-increasing challenges in the clinical diagnoses, treatment options, and new technologies involved in caring for the female patient. The goal of this conference is to increase knowledge and enhance the competence of attendees. Emphasis will be placed on practical application of the evidence-based topics presented.

Topics for this year’s conference have been selected based on the educational needs determined from the identified professional practice gaps. The gaps were derived from a variety of sources including evaluations and outcomes data from past participants; clinical expert input; Symposia Medicus’ online survey; and other medical and nursing data.

Conference format will include didactic lectures, case studies, and question and answer sessions.

An outstanding faculty has been assembled for this conference. Each member was selected based on their individual area of expertise and ability to communicate current, evidence-based, and practical information to participants.

At the conclusion of attending this conference, participants are entitled to a certificate of credit which will be available online. Successful completion of this activity is determined by attending the live activity, review of the educational session materials, and submission of the case studies test, surveys, and attendance verification form. Each participant should only claim credit commensurate with the extent of their participation in the activity.

This educational activity was planned and produced in accordance with the ACCME and ANCC Essentials and Standards relating to continuing medical and nursing education.

Pharmacology Content for Prescriptive Authority
A maximum of 2.25 hours in pharmacology content for prescriptive authority may be earned by attending this educational conference. Sessions awarding pharmacology content for prescriptive authority are denoted with an Rx in the schedule. Please contact your state’s Board of Nursing for specific educational requirements as requirements may vary state to state.

Each registrant will have online access to session materials. The session materials will be available for download approximately two weeks prior to the conference (you’ll receive an email with instructions). If you wish to receive a hard copy of the session materials, please check the box on the registration form in this brochure.

If you are unable to attend this conference, extra copies of the session materials (either paper, flash drive, or digital copy) will be available one week after the conference. CRUISE RATES, DEPOSITS, AND FINAL PAYMENT REQUIREMENTS

All cruise rates quoted are per person, based on double occupancy (U.S. dollars, cruise-only). The price of your cruise includes ship accommodations, taxes, fine dining with all meals included onboard, coffee, tea, milk, juices, and sparkling wine with breakfast, unlimited red and white wines from Europe’s great wine regions, beer, and soft drinks with every lunch and dinner aboard the ship, bottled waters daily in staterooms, Included Tours in every port (see “Tours”), on-demand entertainment in all staterooms featuring free high-speed Wi-Fi internet access, hit Hollywood movies, extensive music library, English language TV stations, and bicycles to explore on your own or on guided bicycle tours (availability based on first come, first served).

A deposit for all stateroom occupants is due to Symposia Medicus at the time of registration ($1,000 per person). Final payments are due to Symposia Medicus by Friday, July 19, 2019. Payments not received by the due date are subject to stateroom cancellation with no refunds.

AmaWaterways® provides complimentary paper cruise documents (one per stateroom) and you will receive them within four weeks prior to cruise. Please provide your best mailing address on the registration form.

CRUISE CANCELLATION & NAME CHANGE POLICY

A full refund will be made for written cruise cancellations received by Symposia Medicus on or before Friday, July 19, 2019. After July 19, 2019, no refunds will be made.

Name changes require the prior approval of AmaWaterways® and are subject to additional fees. Cruise tickets are non-transferable. For security reasons, no name changes are allowed by AmaWaterways® after Monday, October 28, 2019.

PREGNANCY POLICY

If a pregnant guest has entered or will be entering their 24th week before or during the cruise, they are NOT ALLOWED to cruise.

If a pregnant guest is less than 24 weeks, a physician’s “Fit to Travel” note stating that they are in good health, not a high risk pregnancy, and not entering the 24th week must be presented at check-in and carried onboard.

GUESTS UNDER THE AGE OF 18

All guests under the age of 18 are to be in a stateroom with an adult, and must remain supervised at all times, and their safety is the responsibility of the accompanying adult(s). Children must be at least 8 years old by November 7, 2019, to go on the cruise. Denial of boarding for children who do not satisfy this minimum age requirement may also result in the denial of boarding for one or more guests sailing with that child. No refunds will be given as a result of the denial of boarding to an underage child or other accompanying guests. AmaWaterways® does not provide child-specific programs or child-minding facilities.

Children 8-10 years old at the time of embarkation may share a cabin at a rate of 25% of the cabin rate with 2 adults provided the child is able to share the bed with the adults – no additional or rollaway bed will be provided. Children 11-17 years old will be charged the full rate and can share a room with adults in a triple occupancy stateroom (limited). For rates and availability, please contact Symposia Medicus.

PASSPORT AND VISA INFORMATION

Passports are REQUIRED for every passenger. If you do not have a passport, please allow a minimum of 4-6 weeks to receive it after you complete and mail the application. Your passport cannot expire before May 15, 2020 (or 6 months after your final date of travel).

It is the sole responsibility of the guest to identify and obtain all required travel documents and have them available when necessary. These appropriate valid travel documents such as passports, visas, inoculation certificates, and/or family legal documents are required for ship boarding and country entry and/or re-entry into the United States and other countries. Based on citizenship, many countries require an entry visa. Obtaining required visas and any other required travel documentation, and assuring these are complete and correct in all ways, is the sole responsibility of each traveler. AmaWaterways® and Symposia Medicus are not in a position to obtain or verify the accuracy or completeness of any participant’s visa or other required documentation and shall not be responsible or liable for delay or missed portion of any part of the itinerary or other problems resulting from the participant lacking the appropriate travel documentation.

Guests who do not possess the proper documentation may be prevented from boarding their flight or ship, or from entering a country, and may be subject to fines.

Note: No refunds will be given to individuals who fail to bring proper documentation.

AIRLINE TRAVEL INFORMATION

ARRIVAL FLIGHT: You will fly into Budapest, Hungary (Budapest Ferenc Liszt International Airport, airport code – BUD), which is approximately 40 minutes to the cruise port.

It is recommended you allow a minimum of five hours from the time your flight arrives in Budapest to the time the ship departs (at 9:30pm).

DEPARTURE FLIGHT: You will fly out of Munich, Germany (Munich Airport, airport code – MUC), which is approximately 1 hour and 30 minutes from the cruise port in Vilshofen.

Going home, it is recommended you allow at least six to eight hours between when the ship disembarks in Vilshofen and the time your plane departs Munich, Germany (disembarkation starts at approximately 9:00am). The five to eight hours allow you time for ship clearance, transportation to the airport, customs clearance (if applicable), and airline security. You can also request to be one of the first groups to depart the ship by going to the Reception Desk as soon as possible once you board the ship and letting them know that due to a tight airline schedule, you need to be included in the first group off the ship.

IF YOU WANT TO PURCHASE AIR ARRANGEMENTS THROUGH AMAWaterways®:

AmaWaterways® allows you to purchase airline tickets through them. After you are registered and receive a reservation number, we will send you via email, the contact information to make these arrangements. AmaWaterways® will require a photocopy of passports for travelers prior to making airline reservations. Any changes or cancellations may be subject to non-refundable service charges and additional airfare costs.

PRE AND POST CRUISE PACKAGES

AmaWaterways® offers pre/post cruise packages, for an additional cost. After you are registered and receive a reservation number, we will send you via email, the contact information to make these arrangements.

2-Night Pre-Cruise Package – offers 2-nights in Budapest to include hotel stay, breakfast, city tour, and transfer to the ship

3-Night Post-Cruise Package – offers 3-nights in Prague to include transfer from ship, hotel stay, breakfast, and city tours

GROUND TRANSPORTATION

Whether you choose to make your own air travel arrangements or you booked your air through AmaWaterways®, you can purchase transfers to/from the ship (only from Budapest airport or to Munich airport) from AmaWaterways®. Please note rates listed below are subject to change.

Arrival transfers from the Budapest airport to the ship on November 8, 2019 are $30 per person and run throughout the day.

Departure transfers to the airport in Munich on November 15, 2019 are $30 per person and run throughout the morning and early afternoon.

After you are registered and receive a reservation number, we will send you via email the contact information to make these arrangements.

Ground transportation reservations must be made through AmaWaterways® by Friday, September 27, 2019. Your flight itinerary must be provided in order to confirm your transfers.

ALCOHOL ABOARD SHIP

During the cruise, you are offered complimentary sparkling wine with breakfast, unlimited complimentary beer, wine, and soft drinks with lunch and dinner. Any additional drinks (cocktails) not during dining times, are available at an additional fee, per drink at the bar and charged to your stateroom.

DINING ABOARD SHIP

AmaWaterways® expert chefs craft menus that feature exquisite, locally-inspired cuisine as well as traditional, Western offerings. At breakfast, sip sparkling wine as you partake in the buffet with fresh pastries, or opt for something made-to-order from the menu. For lunch head to the restaurant for full menu service and high-quality regional wine or beer. Dinner is a multi-course affair with wine recommendations that accompany visually stunning, delicious dishes.

The Executive Chef, Primus Perchtold, is the winner of three gold medals at the European Championship of Culinary Art and oversees the menus aboard every AmaWaterways® cruise in Europe. You will find one of his recipes on your pillow each night, just in case you want to recreate his culinary magic at home.

Special dietary requests must be provided to Symposia Medicus at time of reservation. An effort will be made to meet the request, but AmaWaterways® cannot assure the request can be accommodated.

TOURS

A sightseeing tour is included in each city or town you visit, along with free time to explore the city according to your own interests. AmaWaterways® offers the below:

Included Tours – Standard tours offered in each port. These are included in the price of the cruise. Pre-registration is not required, but it is recommended. Included tour registration is available online approximately 60 days prior to embarkation and once online check-in is completed.

Special Interest Tours – Tours offered in place of the standard tours in each port. These are included in the price of the cruise. Pre-registration is not required, but it is recommended (approximately 60 days prior to embarkation and once online check-in is completed), with the exception of bike tours. Bike tours are only available to book once onboard, on a first come, first served basis.

Optional Tours – Tours offered as an option at an additional cost. These are in addition to the standard tours offered in each port and can only be booked once onboard, on a first come, first served basis.

During all the tours, AmaWaterways® provides a sophisticated audio headset system that allows guests to comfortably listen to the tour guide while taking photos and admiring some of Europe’s hidden gems.

CONFERENCE CANCELLATION POLICIES

Conference cancellations must be made in writing (via email or mail) to Symposia Medicus and a refund, less a 10% processing fee, will be given. No refunds will be made if cancellation is received after Friday, July 19, 2019; however, tuition, less 10% processing fee, may be applied to a future Symposia Medicus conference.

In the unlikely event of conference cancellation by Symposia Medicus, full tuition and, if reservation was made with Symposia Medicus, stateroom payment refunds will be made. Symposia Medicus will not be responsible for refunding travel or any other expenses.
